In some people the discrepancy between the upper and lower jaws is so marked that braces or clear aligners alone are not enough. In such cases, orthodontic treatment is planned together with orthognathic surgery (jaw surgery). The aim is not only for the teeth to bite correctly, but also to help the jaws achieve a more balanced relationship with each other and with the face as a whole.
It is considered in cases such as a markedly retruded or protruded lower or upper jaw, open bite, facial asymmetry, difficulty chewing and bite problems that cannot be corrected with orthodontics alone. It is usually carried out in adults once growth is complete. Not every discrepancy requires surgery; this decision is made after examination and investigations.
Treatment is carried out in cooperation between the orthodontist and the jaw surgeon. The process usually has three stages:
1. Pre-surgical orthodontics: The teeth are prepared so that they will bite correctly after surgery.
2. Surgery: Jaw surgery is performed by the relevant specialist surgeon.
3. Post-surgical orthodontics: The bite is fine-tuned, and then retention (a retainer) begins.
Planning is based on examination, cephalometric and panoramic X-rays, a digital scan and photographic records. The jaw joint is also included in the assessment. At our clinic these records are taken in the same appointment.
This is a long-term treatment that includes stages before, during and after surgery. Duration and details vary from person to person and are discussed openly at the examination. We do not guarantee results, but we explain what to expect at every stage.
Is surgery essential? Not for every jaw discrepancy; the decision is made at the examination.
Who performs the surgery? The relevant specialist jaw surgeon. The orthodontic plan is ours; the surgical procedure is carried out by the surgical team.
At what age is it done? It is considered in adults, once growth is complete.
